Lakeview Center, Inc. (LCI) knows the job market is competitive, which is why we offer a robust and comprehensive benefits package to help you and your family throughout your work and personal journey. In addition to offering voluntary benefits like medical, dental, and vision plans, we also provide employer-paid benefits, including life insurance, long-term disability and paid time off.

LCI Benefits Eligibility: A full-time, benefits-eligible team member is any team member whose position is classified as full-time and who regularly works a minimum of 30 hours per week. Benefits are effective on the first of the month, after 30 days of employment.

Medical Plans

We offer two major medical plans through Blue Cross Blue Shield with premium discounts available to those who participate in our wellness program.

  • Predictive Cost Health Plan (PPO) – This plan is our traditional plan designed with set copays for each service rendered. With the PPO plan you get access to Teladoc at no cost and generic prescription medications free at the Lakeview Center Pharmacy.
  • High Deductible Health Plan (HDHP) – This plan has an annual deductible that once met, the coverage would then pay 80% of in-network covered services until the out-of-pocket maximum has been reached. With this plan, team members are eligible to elect a Health Savings Account to further assist with health cost savings.

LCI Pharmacy Benefits

Lakeview Center Pharmacy is located on the main campus, and is a full-service pharmacy. Team members can obtain deeply-discounted over-the-counter medications, transfer existing prescriptions, and request refills online or via the mobile app at mobilRx pharmacy. To make getting your prescriptions even more convenient, mail order is available to refill prescriptions and have them sent right to your mailbox. Additionally, courier services are delivering prescriptions to Lakeview locations such as Defuniak Springs, Shalimar and Milton.

Teladoc is a service covered under the LCI health plans that serves as a convenient alternative for non-emergency medical care. Teladoc provides 24/7 access to a physician right from your computer or mobile device.

Employee Assistance Program

Free, confidential counseling and resources help with issues like depression, substance misuse, work/life balance and child care. Financial counseling also is available.

Dental Plans

Two dental plan options (Mid & High), both of which allow qualifying members to carry over part of their unused maximum benefit to the following year. The dental plans offer free preventive and diagnostic screenings as well as discounts on other in-network services (including child orthodontia).

Health Savings Account (HSA)

An HSA is a tax-exempt account exclusively for paying qualified medical expenses. (Team members must be enrolled in the HDHP to utilize this benefit.)

Pet Insurance

Nationwide Pet Insurance provides health care coverage for your dog, cat, bird, hamster or other exotic pets.

Employer Paid Benefits:

Basic Life Insurance and AD&D

Be comforted knowing you have Basic Life and Accidental Death and Dismemberment insurance coverage valued at twice your annual salary (minimum $50,000 - maximum $300,000).

Long Term Disability

A long-term disability plan pays participants 50% of their monthly income (up to $5,000 per month) following a 90-day waiting period.

Defined Contribution 403(b) Retirement Savings Plan

Our 403(b) retirement savings plan is managed by Voya. Once eligible, LCI will match 50-cents on the dollar up to 6% of your contributions, with a maximum employer contribution of 3%. Full-time team members are eligible for matching contributions after one year of employment and after working 1,000 hours within the year.

The PerkSpot Team Member discount program provides a web-based platform to display online discounts from hundreds of retailers

Travel Assistance

Team Members can receive help with things like visa and passport requirements, pre-travel immunization information, emergency travel arrangements and prescription replacements

Lakeview Center, Inc. Paid Time-Off Program:

Paid Time Off

(PTO) provides eligible team members with paid time away from work for: Lakeview Center recognized holidays, vacation, personal time, personal illness or time off to care for dependents.

Paid Time off (PTO) Eligibility:

All full-time, benefits-eligible team members earn paid time off. PTO is not earned by part-time, relief, on-call, temporary or contract employees or those whose job classifications exclude PTO and other employee benefits. Eligible staff working less than 40 hours will have their PTO accrual rate prorated accordingly.

New Hires:

Full-time benefits eligible team members will be permitted to accrue a negative PTO balance during their 90-day new hire probationary period to cover leave pay for Lakeview Center recognized holidays that fall within their first 90 days of employment. PTO time will be deducted from future accruals until the PTO balance is no longer in the negative. Negative accrual balances during the first 90 days are not allowed to cover for sick or personal days off.

PTO Accrual Rates:

PTO is earned every pay period and credited to a team member’s PTO bank on the last day of each pay period. Team members are eligible to use their PTO balance upon successful completion of a 90-day probationary period with the exception of holidays falling within an employee’s first 90 days of employment. PTO is earned according to the following schedule:
