BHC Mammogram Fund
Baptist Hospital provides "Free Screening Mammos" for uninsured women within the community during Breast Cancer Awareness Month. This program is funded by donations from the community and or other local fund raising events. To qualify for a free mammo, patients must meet the following criteria:
- Patient must have a Primary Care Physician
- Patient must be over the age of 35
- Patient's last mammogram must be over 12 months
- Patient must have had at least 5 years cancer free screenings ( No Diagnostic Screening will be done in this program)
- Patient must not be currently experiencing any Breast problems
- Patient must be uninsured - self pay
- Patient must meet or be at least 200% above Federal Poverty Guidelines (Patients must completed Financial Assistance Application, along with providing proof of income, proof of Food Stamps eligibility, prior year tax returns and 2 most recent bank statements, plus any other required documentation to proof financial hardship.
- Upon meeting this criteria and as long as we have spaces available (funding available) patient would qualify for a Free Screening Mammo.
